Can the same teacher fill my teacher evaluation form and my school report if he/she knows me best.
			

The answer depends on what the instructions say on the application.  Many applications explicitly request that all your recommendations come from different individuals in a capacity that is designated.  However, yours might be different, so please read it over carefully, and when in doubt feel free to contact the office directly with any questions.  That being said, we do believe that it is better to have the recommendations come from different people as opposed to multiple recommendations from a single person who knows you better.  After all, the more people you have to endorse you, the larger and more varied the scope you give the school to judge you by.
